- Fotocopia della tessera sanitaria – Photocopy of the health card
- Certificato medico d’idoneità sportiva (non scaduto) per la disciplina praticata; per i non agonisti di un certificato di sana e robusta costituzione rilasciato dal medico di base – Medical certificate of sports fitness (not expired) for the discipline practiced; for non-agonists of a certificate of healthy and robust constitution issued by the general practitioner
- Nulla osta da parte della società di appartenenza – Clearance from the club to which they belong;
- Fotocopia del documento d’identità valido – Photocopy of valid ID identity document;
- Green pass in corso di validità – Valid green pass
